Required Football Forms
ALL football players must have the following forms completed BEFORE picking up equipment.
- Birth certificate or valid passport (first year players only)
- agw medical treatment consent and information form_updated 5.29.24
- AGW-Medical-Clearance (1)
- Equipment_2026 with check for deposit
Required Cheer Forms
ALL cheer athletes must have the following completed BEFORE equipment is dispersed.
